Fred Goers, Business Office Manager

Fred GoersFred Goers is the Business Office Manager of Colfax Health and Rehab. He takes on many responsibilities within the facility and does a great job in doing so. In his spare time, Fred enjoys building wood projects and decks, remodeling his old house, and working with computers. He has two sons, one of whom is teaching in Cambridge, and the other who works at Value Implement where Fred's wife is the co-manager.

One of the greatest things about working here at Colfax Health and Rehab is having the advantage of working with diligent, friendly people, as Fred confirms he is fortunate to be able to work with people he has known for years.

Penny Streifel, Director of Activities

Penny StreifelPenny Streifel is the Activities Director. Penny has worked at Colfax Health and Rehab for 12 years. She began as a nursing assistant, in the Restorative Department. She has been the activity director for three years. Penny is originally from Minnesota.

Penny considers the Residents at Colfax Health and Rehab her extended family. My goal is to help make this chapter of the “Residents Book” filled with joy and happiness. Some of Penny’s hobbies are “Oil Painting” and “Photography”.


Jill Gengler, Administrator

Jill Gengler is the Administrator at Colfax Health & Rehab. She joined the organization eight years ago and is honored to be part of an organization with such a long standing reputation for quality and caring. "The main focus of my position will always be to maintain and uphold our reputation through the many changes presented to us in the health care industry."

She currently resides in Bloomer with her husband Steve and their three beautiful daughters, Lauren, Stephanie and Jessica. Jill is originally from New Auburn, Wisconsin and attended UW-Eau Claire where she received her degree in Health Care Administration. She worked in Milwaukee for a large Health Care Corporation before relocating back to this area.



Laurie Craft, Resident Service Director & Admissions Coordinator

Laurie CraftLaurie Craft is the Resident Service Director and Admissions Coordinator. Laurie has worked in the resident service field for over twenty years, whereby ten of her first years were spent in Mesa, Arizona as a social worker and consultant. She decided to come back to the Midwest in March of 2000 where she currently resides on Tainter Lake. Some of Laurie's hobbies include gardening, fishing, and stained glass art. She loves to spend time with her family and is known for her warm smile and making people laugh.

Laurie is proud to claim that Colfax Health and Rehabilitation Center offers more than just health care. "We are a small, family-oriented facility that dedicates our heart and soul to forming and maintaining long-term friendships with our staff, residents, and their families. It's a great feeling knowing that our entire staff has the ability and opportunity to make a difference in our residents' lives each day." - Laurie


Terrie Sommer, Quality Assurance Consultant

Terrie SommerTerrie joined the team at Colfax Health and Rehab in April 2011 as a nurse consultant after retiring from
the State of Wisconsin as a nurse surveyor, surveying long term care facilities for almost 17 years. During the past 9 years Terrie also worked weekends as a charge nurse in hospital in Hastings, Minnesota working in ICU and on medical surgical units. Terrie has been married to her husband for 38 years; they have 3 children and 6 grandchildren.

Some of Terrie’s hobbies include being with her family, especially her grandkids, going to auctions, thrift sales, cooking, baking and casino trips. During the summer they spend a lot of time at their lake home in Gordon Wisconsin.



Richard VanDaalwyk, Director of Maintenance

Richard VanDaalwykRichard VanDaalwyk is the Director of Maintenance, and has been for nearly thirty years. He grew up in Kimberly, Wisconsin, married his wife of thirty-eight years, and has two children and one grandson. In his spare time, Dick likes gardening, hunting, fishing, and kayaking.

Aside from taking advantage of free time and doing the things he loves, Dick spends a great deal of his time here at Colfax Health and Rehabilitation Center. He is grateful for working with a facility that emphasizes a relaxing, family-oriented atmosphere. "I like the people, and I enjoy it here. Even if I can't solve a problem, I do my best to find the problem. We do all we can to make people comfortable." --Dick



Teresa Wadell, Director of Nursing

Teresa WadellTeresa Wadell is a licensed RN and the Director of Nursing here at Colfax Health and Rehabilitation Center. Teresa was a former dairy farmer, but had a change of career path later in life. First as a CNA here at Colfax Health and Rehab, Teresa felt welcomed and supported. Teresa is married and a proud parent of eight beautiful children. She loves gardening, camping, and her three big dogs.

Teresa finds comfort in the fact that she is part of a place she knows she can send any of her loved ones someday. Teresa felt so welcome right away. She received help from her peers whenever she needed it - even when she didn't ask for it. "This is not just a job; it is a place of growth and opportunity for both our employees and our residents. I love what I do because I love the long-term relationships with people. I like to see the results of our work; it's very rewarding." - Teresa


Tina Logslett, Director of Housekeeping and Laundry

Tina LogslettTina is the Housekeeping Supervisor for Colfax Health and Rehab. Although originally from Milwaukee, Tina has lived in Colfax for almost her whole life, where she later met and married her husband of twenty-nine years. She and her husband have three beautiful children and a grandson, Lucas.

For six years, Tina has been a significant attribute in keeping the facility well maintained and making residents and other staff members feel comfortable and happy with their time here. Her vibrant personality and diligent work ethic are the perfect combination for success as a leader.



Barb Stobb, Health Information

Barb StobbFormer Administrative Assistant, Barb Stobb has taken over the role of Health Information. She has been employed at Colfax Health & Rehab since 1977. Starting in the Dietary and Activity Departments and taking on the Administrative Assistant role in 1988.

Barb and her husband of 38 years, Paul live south of Colfax in what is called Bear Valley. There they have a small hobby farm, and enjoy feeding all of the local wildlife that visits them daily. Their children, Rebecca and John, are grown and have blessed them with 4 beautiful grandchildren.

Away from work, Barb enjoys spending time with her family, camping and 4 wheeling, crafting, crocheting, gardening and canning.

Barb has treasured her many years of service at Colfax Health and Rehab and fondly remembers all of the residents and their families she has befriended. She has seen many changes through out the 35 plus years and is looking forward to what the future holds, especially the move to our new facility.

Health Information is an always changing department, but Barb is up to the challenge, and looks forward to what each new day will bring.

Amy Ralph, RD - Registered Dietician

Amy RalphAmy is the Food and Nutrition Services Director. She joined the organization over one year ago as the facility's Registered Dietitian. "The part that I enjoy most about my job is the opportunity to get to know the residents and help them reach their best possible health through nutrition."

She currently lives outside of Colfax with her husband Jon and their two active sons, Eli and Sam. Amy is originally from Wadena, Minnesota and attended UW-Stout where she received her Bachelor degree in Dietetics and her Master degree in Food & Nutritional Sciences. Her hobbies include being outdoors, spending time with family and friends, and camping.
